Do not use the website, ordinary email, downloaded forms, fax, or a registration request for an emergency. Call 911 or go to the nearest emergency department. Call the appropriate clinical office for a time-sensitive concern.

Appointments, referrals, and courses

A web page, fax, form, referral, or course-registration request does not confirm acceptance for care, an appointment, enrollment, availability, or payment. Confirmation must come directly from authorized practice staff.

No guarantee of coverage or suitability

Site content does not guarantee insurance coverage, network status, authorization, payment, outcome, or that a service is appropriate for a particular patient or care setting. Services, schedules, clinicians, course terms, and availability may change.

Downloaded material

Patient forms are provided for convenience. Patients remain responsible for reviewing completed forms and submitting them through an approved channel. A downloaded form may be stored by the device, browser, cloud service, or PDF application selected by the user.
